1. Initial Assessment: In-Person Home Visit for Home Care OR Online Visit for Holistic ServicesA health professional conducts a systematic, dynamic assessment to gather and analyze data about the client. This assessment includes not only physiological information but also psychological, sociocultural, spiritual, economic, and lifestyle factors.Please share any relevant information regarding your condition. All data collected will be kept confidential and used solely for the development of your care plan.While many nursing tasks can be delegated to unregulated healthcare professionals under the supervision of nursing staff, certain tasks must be performed exclusively by a registered nurse. The nursing professional determines these tasks during the home assessment.

2. DiagnosisA nursing diagnosis is the nurse’s clinical judgment regarding the client’s response to actual or potential health conditions or needs.While a medical diagnosis addresses specific diseases or medical conditions, a nursing diagnosis focuses on the human response to health problems and life processes.For example, a nursing diagnosis might not only acknowledge that you are in pain but also recognize the accompanying issues, such as anxiety, poor nutrition, family conflicts, or the potential for complications.

3. Planning (Done Remotely)Based on the assessment, psychosomatic analysis, and nursing diagnosis, we will develop and share the goals of care. These goals are measurable, achievable, and include both short- and long-term objectives. The process may elicit emotional and physical responses, and we may employ techniques like emotional freedom techniques (EFT) to help you navigate these. Several planning sessions may be required.

4. ImplementationCare is provided according to the developed care plan.You will receive a clear, step-by-step implementation guide, which is critical for ensuring sustained change. Caregivers are trained and supervised, both directly and indirectly, during this stage. The frequency of nursing visits will depend on your condition and care needs.

5. EvaluationBoth your condition and the effectiveness of the care plan will be continuously monitored and adjusted as needed. Upon discharge from nursing or caregiver assistance, we will meet to evaluate the results.Home care can be either short-term or convalescent. Convalescent care provides a home-like environment during recovery from surgery, injury, or illness, and can also serve as a transitional care option following a stroke or long-term illness.
